Family Assessment and Planning Team (FAPT)
The Children’s Services Act was created to ensure that at-risk youth and their families have access to as much support needed to transform successfully and have better futures. The Family and Assessment Planning Team (FAPT) is one of two teams in Fluvanna County that is responsible for the operation of the CSA in Fluvanna County.
Purpose: The purpose of the FAPT is to assess the strengths and needs of at-risk youths referred, determine if these youths are eligible for funding through the CSA for needed services, and determine the best services that are needed for these youths to transform and succeed.
Membership: Membership shall meet the requirements of the Code of Virginia § 2.2-5207, and is appointed by the Community Policy and Management Team. Agencies represented are permanent members, and may have multiple representatives providing input during assessments and deliberations, although there will be only one voting representative from each agency. These agencies include:
- Fluvanna Department of Social Services
- Fluvanna County Public Schools
- Fluvanna Court Services Unit
- Region Ten CSB
In addition, the following representatives are also included, who each serve a maximum of two 2-year terms:
A member of the community to represent parent and families served by the CSA, who is a parent/guardian of a child currently enrolled in the Fluvanna County public school system, and/or has received services funded by the CSA.
A representative of a private organization that currently receives funding from the County of Fluvanna CSA to provide children’s or family services.
